Privacy
The privacy of my clients is very important to me. The picture on this website in which you see a client is therefore not of a real client, but instead it has been staged. This website also doesn't make use of cookies.
I do my best to ensure your privacy. This means I handle your personal and medical records with care and I ensure that unauthorized people do not gain access to your records.
As the therapist responsible for your treatment I have sole access to you records. I am required to treat all information you give me with confidentiality.
In order to provide good treatment, I have to maintain records of said treatment. This is also mandatory under law (WGBO;
wet op de geneeskundige behandelovereenkomst). Your records contain notes about your condition and information about the provided treatments. You are entitled to view your own files on request.
Part of the notes I make during a treatment are meant for my personal use. On request I can make a report of these for a moderate fee.
Your records may also contain information, necessary for your treatment, that has been provided to me by other providers of care, for instance your general practioner. I will only request information from your general practitioner or any other caregivers with your explicit consent.
Information from your records can and will also be used for the following purposes:
- - To inform other providers of care; for instance after ending your treatment or on referral to another therapist. This will only happen with your explicit consent.
- - On your explicit request I can provide your information to a substitute therapist during my absence.
- - In case something serious happens to me as a therapist, rendering me not only unable to fulfill my appointments, but also to inform my clients about that personally, my substitute therapist will get access to the necessary information (names, phone-numbers and e-mailaddresses) to inform my clients about the situation.
- - To discuss anonymized with colleagues in for instance intervision.
- - General information from your records (adress information, dates of appointments) are used by me for the financial administration so I can send you an invoice. In case an invoice is still not paid to me after repeated reminders I can share this information with a debt collection agency.
If I want to make use of your records in any other way or for any other reason, I will inform you about this and ask for your explicit consent.
As stated in the WGBO these records will be kept for a period of 20 years.
The invoice you receive will contain the following data, so you can send the invoice to your insurance company and ask for a refund, depending on the conditions of your policy.
- - your name and address
- - your date of birth
- - the date(s) of treatment
- - a short, generic descripton of the treatment (for instance 'therapy session')
- - the costs of the treatment
This invoice can be sent to you by mail or by e-mail, at your request. In case you ask for the invoice by e-mail, the possibility of data-leakage through the use of electronic mail is at your own risk.
